Do you receive one or more pension payments from one Member State of the European Union?
To receive reimbursements for your health care costs in Belgium you must request an S1 or E121 form from your health care insurer in your home country. When you receive this form, you must send it to Symbio along with a registration form and a copy of your Belgian identity card. Please note that only original documents will be accepted.
Upon receipt of your application, we will send you identification stickers.
If you have a Belgian national registration number, you can use your eID at the chemist’s, at the hospital or at some care providers’. You will be able to benefit from the third-party payment system when it’s possible. If you don’t have a national registration number, we suggest that you send us a request so that we can order an ISI+ card for you. This card will allow you to benefit from the same advantages than the eID at the chemist’s, at the hospital, or at care providers’ who use a card reader.
If your spouse or partner does not have mutual insurance cover in Belgium yet and has no income, you can register them as a « dependant ». This means that their entitlement to health care will be dependent on your file. They may register on three conditions:
- you must not have a spouse or partner already registered as a dependant;
- the dependant must have an income below € 2,326 per quarter;
- you must be registered with the same ‘household composition’ (composition de ménage).
A document must be completed by the spouse or partner and the person requesting their registration.
You must also ask your health care insurer in your home country that issued your E121 or S1 form to send you another one in the name of your spouse or partner.

The procedure above also applies in this case. The E121/S1 form must be issued to you by the social security institution of the country in which you had the longest professional career.
Do you receive multiple pension payments from various countries, including Belgium?
You are entitled to receive reimbursements for your health care costs in Belgium as a ‘Belgian pensioner’. To do this, you must send Symbio a ‘certificate of pension for use for health care purposes’ (attestation de pension devant servir en matière de soins de santé) along with a registration form.
